ORGANISATIONAL REGULATIONS
DUO MEDICA ANNA SOBCZYŃSKA

 

  1. Legal basis for operation

§1

These Organizational Regulations, hereinafter referred to as the Regulations, define the organization and order of the process of providing health care services at DUO MEDICA Gabinety Lekarskie, located in Katowice, hereinafter referred to as “DUO MEDICA Gabinety Lekarskie” or the “Facility”, and in particular:

  • The course of the process of providing health care services with ensuring the proper availability and quality of these services in the organizational units of DUO MEDICA Gabinety Lekarskie,
  • organization and tasks of individual organizational units of DUO MEDICA Gabinety Lekarskie, including the scopes of conditions for cooperation between these units to ensure the efficient functioning of DUO MEDICA Gabinety Lekarskie in terms of treatment, administration and economy,
  • the conditions for interaction with other health care providers in ensuring the correctness of diagnosis, treatment of patients and continuity of treatment.

§2

  1. DUO MEDICA Gabinety Lekarskie is a medical establishment of a medical entity – Anna Sobczynska conducting business under the name DUO MEDICA Anna Sobczynska, based in Katowice.
  2. Anna Sobczynska, as an entrepreneur, is registered in the Central Registration and Information on Business Activity conducted by the Minister of Development.
  3. The treatment entity is registered in the Register of Entities Performing Medical Activities kept by the Silesian Governor under the number 000000025694.
  4. The head of the treatment entity is Anna Sobczyńska.
  5. The therapeutic activity is carried out at the headquarters of the Department in Katowice at ul. Grabowej 1d, 40-172 Katowice. The place of providing health services is the headquarters of the Facility. In justified cases, services are provided at the patient’s place of residence.

§3

The provisions of the Regulations apply to all persons cooperating with DUO MEDICA Gabinety Lekarskie, as well as persons performing health services for DUO MEDICA Gabinety Lekarskie on the basis of civil law contracts, patients of DUO MEDICA Gabinety Lekarskie, persons accompanying the aforementioned. patients and visitors.

§4

DUO MEDICA Medical Clinics operates on the basis of generally applicable laws and internal regulations, including in particular:

  • Law of 15.04.2011. On therapeutic activity (i.e. Journal of Laws of 2020, item 295),
  • Law of 6.11.2008. On Patients’ Rights and Patients’ Ombudsman (i.e., Journal of Laws of 2019, item 1127, as amended),
  • of these Regulations,
  • Internal orders of the Manager of DUO MEDICA Medical Clinics.
  1. Objectives and tasks of the entity

  • provision of health services by medical specialists,
  • forecasting and organizing preventive and curative activities,
  • providing individual advice and consultation,
  • prganizing and conducting health education,
  • applying for the supply of aids and orthopedic items,
  • interaction with other organizational units of health care, schools, workplaces, organizations and associations,
  • adjudication of unfitness for work,
  • adjudication and opinion on health status,
  • other tasks to improve the process of providing health services and ordered by the Head of DUO MEDICA Medical Offices.
  1. The primary purpose of the Department’s activities is the therapeutic activity of preserving, saving, restoring or improving health and other medical activities resulting from the treatment process.

  1. In the structure of the Establishment is distinguished:
  • organizational units of primary activities – performing tasks in the provision of health services,
  • administrative, economic and technical cells – ensuring proper service and operation of the Plant.

  • Gynecology outpatient clinic,
  • Surgical Outpatient Clinic,
  • Treatment room,
  • Endocrinology outpatient clinic,
  • Aesthetic medicine office,
  • Dermatology outpatient clinic,
  • Pediatric surgery outpatient clinic,
  • Dietary Clinic,
  • Vascular surgery outpatient clinic,
  • Diabetes outpatient clinic,
  • Radiology Laboratory,
  • Plastic surgery outpatient clinic.
  1. The primary activities of the Department include:

The facility conducts therapeutic activities in the type of outpatient health services.

  1. DUO MEDICA Medical Clinics organizes health care services in the Department of dentistry, gynecology, surgery, endocrinology, aesthetic medicine, dermatology, pediatric surgery, vascular surgery, plastic surgery, radiology, diabetology, and dietetics.
  2. In justified cases, services are provided at the patient’s home.
  3. Patient registration is carried out both in person (or by family members or third parties), by phone and electronically, with the day and time of the health service designated.
  4. Health services are provided on the day of application or at a time agreed upon
    with the patient.
  5. The facility provides paid health services, and does not provide free health services to persons insured under the National Health Fund.
  6. The Department provides health care services to patients who have group insurance or other insurance that entitles them to receive services at the Department on the basis of agreements between the Department and the insurer (cooperating partner).

  1. Doctors providing health services at the Department, if necessary, refer treated patients for required radiological and laboratory tests and medical consultations in other specialties.
  2. Adult and minor patients presenting for treatment at the Facility are not required to have referrals, but may have a referral from another doctor.

  • Dental Clinic,
  • Gynecology outpatient clinic,
  • Surgical Outpatient Clinic,
  • Treatment room,
  • Endocrinology outpatient clinic,
  • Aesthetic medicine office,
  • Dermatology outpatient clinic,
  • Pediatric surgery outpatient clinic,
  • Dietary Clinic,
  • Vascular surgery outpatient clinic,
  • Diabetes outpatient clinic,
  • Radiology Laboratory,
  • Plastic surgery outpatient clinic.

§11

The primary tasks of the Dental Clinic include:

  • treatment of children in the field of conservative dentistry, prosthodontics, orthodontics,
  • adult treatment of conservative dentistry, prosthodontics, orthodontics, endodontics,
  • treatment of children and adults in the field of dental surgery,
  • taking x-rays.

The primary tasks of the Gynecology Outpatient Clinic include:

  • providing advice on gynecology and obstetrics,
  • performing minor procedures in gynecology and obstetrics.

The primary tasks of the Surgical Outpatient Clinic include:

  • providing advice on surgery
  • performing minor surgical procedures.

The primary tasks of the Treatment Room include:

  • Perform minor procedures in gynecology and obstetrics,
  • performing minor surgical procedures.

The primary tasks of the Endocrinology Outpatient Clinic include:

  • providing advice on endocrinology

The primary tasks of the aesthetic medicine office include:

  • providing advice and treatments in the field of aesthetic medicine

The primary tasks of the Dermatology Outpatient Clinic include:

  • providing advice on dermatology

The primary tasks of the pediatric surgery outpatient clinic include the following

  • providing advice on pediatric surgery,
  • Performing minor surgical procedures on children.

The primary tasks of the Dietary Clinic include:

  • diet and nutrition counseling,
  • promotion of healthy lifestyle

The primary tasks of the Vascular Surgery Clinic include:

  • providing advice on vascular surgery,
  • performing minor vascular surgery procedures.

The primary tasks of the Diabetes Outpatient Clinic include:

  • providing advice in the field of diabetology.

The main tasks of the Radiological Laboratory include

  • taking adjacent x-rays,
  • taking pantomographic x-rays.

The main tasks of the Plastic Surgery Outpatient Clinic include the following

  • providing advice on plastic surgery,
  • performing minor plastic surgery procedures.
  1. Conditions of cooperation with other entities performing medical activities in ensuring the correctness of diagnosis, treatment, care and rehabilitation of patients and the continuity of the process of providing health services

  1. DUO MEDICA Doctors’ Clinics shall maintain, store and make available medical records of persons receiving health services in accordance with the rules set forth by law.
  2. The facility makes medical records available to other entities performing medical activities and authorized bodies on the basis of the law.
  3. The release of documentation is carried out in a manner that ensures confidentiality
    and data protection.

The plant interacts with other entities on the basis of agreements.

VII. The amount of fees for providing medical records

There is no charge for providing medical records to DUO MEDICA Medical Offices.

VIII. Organization of the process of providing health services in the case of charging

  1. In the case of a fee-for-service health care service, the patient has the right to know the price of that service before it is provided.
  2. Paid health services are provided on the basis of individual billing to the patient, subject to paragraph. 5.
  3. In the case of individually financed services, the patient has the option of paying for the service by making an advance payment – before the service is provided and paying the remainder of the price – after the service is provided. Payment may be made in cash, by payment card and by wire transfer to a designated bank account, subject to paragraph. 5.
  4. At the patient’s request, a VAT invoice is issued each time a paid health service is provided.
  5. The establishment works with partners (insurance companies), which finance health care services to patients according to the terms of the contracts concluded between the parties (the catalog of services available to the patient under the insurance depends on the insurance variant the patient is entitled to).

The amount of fees for health care services performed at DUO MEDICA Medical Offices is specified in Appendix No. 1 to the Regulations. On the website under the Price List tab.

  1. The way the unit is managed

§26

  1. The plant is managed by the Manager and represents it externally.
  2. The manager independently makes decisions regarding DUO MEDICA Medical Clinics and is responsible for them.
  3. In the absence of the Manager, he is replaced by a person authorized by him.
  4. The duties and powers of the Manager include, but are not limited to:
    • organizing and directing the work of subordinate personnel,
    • ensure the rational use of working time and supervise the observance of discipline of subordinate personnel,
    • deciding on matters of employment, remuneration, punishment and dismissal of the Department’s personnel.
